
1. Assess the Injury
Seek first aid or call for help depending on the severity of the injury.
2. Notify Your Employer of your Injury
I recommend telling your employer of your injury immediately, but it must be done within 7 days.
Complete a C-1 Form (Notice of Injury or Occupational Illness Form): Briefly describe accident or circumstances of occupational disease.
3. Seek Medical Attention
Ask your employer what doctor to go to -or- go to any urgent care and tell them it was a work injury.
Complete a C-4 Claim Form (Initial Injury Compensation Claim)
4. Consult with a Workers Compensation Attorney
